Center Well Home Health is seeking a Home Health Physical Therapist to plan and administer prescribed physical therapy treatments in patients' homes, reporting to the Clinical Manager to attain the highest level of physical function.
You will assess patients, develop rehabilitation programs, guide PTAs and Home Health Aides, document progress, and communicate with physicians and families. A current unrestricted PT license and CPR are required; one year of experience is preferred.
