This role is designed to support Digital Transformation within the Southport and Formby Primary Care Network (PCN).

It will involve working centrally at our main Southport office suite, as well as visits to the fourteen practices within Southport and Formby.

The post will support several corefunctions:

• Digital transformation
  :
  Support the PCN and its practices in utilising digital solutions such as online consultation systems and digitised workflows to improve efficiency and service delivery.
• PCN services
  :
  Enable PCN services to digitise workflows and utilise data more effectively to better understand and optimise their services.
• Digital health support
  :
  Assist practices in meeting national and local contract requirements by providing digital health support for both practices and their patients.

This role is advertised as full time but part-time applications will be considered. Hours of work will be agreed between 8am and 6pm Monday through Friday.

• EMIS and Digital Systems Support
• Collaboration with PCN management teamsand staff across internal and external teams to drive digital transformation and service improvements
• Patient Support and assisting practices with Health Inequality Reduction
• Drive Service Efficiency by standardising processes and workflows
• Project and Task Management

You will be working in a growing team consisting of both clinical and non-clinical colleagues, so teamwork and good communication skills are vital.

Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.

Southport and Formby Health work collaboratively with Southport and Formby Primary Care Network (PCN).

Southport and Formby PCN is a group of 14 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and wellbeing for our local population of around 129,000 people.

We work hard to ensure that the services we offer are the best that they can be and we are constantly developing to ensure we are providing the best services for ourlocal community.

We are seeking a passionate and self-motivated individual with a strong understanding of general practice data to join our growing team. This role will play a key part in using digital solutions to develop our data management capabilities, supporting our member practices, and contributing to the delivery of the PCN's long-term Digital and IT strategy.

Duties to include the following;

EMIS and Digital Systems Support:

• Create and run searches using EMIS to extract and analyse relevant data.
• Develop and maintain templates, protocols, and concepts inEMIS to streamline practice and PCN workflows.
• Utilise digital clinical tools such as Ardens, iPlato, and Accurx to enhance digital healthcare delivery across practices and the PCN.
• Use Microsoft Office 365 tools for advanced analytics, reporting and visualisation to improve and streamline service managements andreduce dependence on manual reporting and ad hoc fixes.

Digital Transformation via Collaboration and Communication:

• Collaborate with PCN management teams and staff across internal and external teams to drive digital transformation and service improvements including working on identified projects to support with data quality/reporting and evaluation.
• Building and maintaining relationships with practices by keeping them up to date on performance measures such as the Sefton Local Quality Contract (LQC) and Investment Impact Fund (IIF). This will involve supporting and monitoring performance and reporting this back regularly to key stakeholders.
• Collaborate with existing PCN services, to drive the digital maturity across the network.
• Attending relevant meetings for services across the organisation.
• Identify opportunities for service improvement through digital opportunities.

Care Coordination / Patient Support and Health Inequality Reduction:
