OverviewAt United Healthcare, we’re simplifying the health care experience, creating healthier communities and removing barriers to quality care. The Nurse Practitioner (NP) holds primary responsibility to conduct clinical assessments of plan members, document encounters in the authorized electronic medical record system, and facilitate optimal health outcomes through cost-effective care. The NP reports to the NP Manager.
Primary Responsibilities- Conduct assessments in member homes and clinics with emphasis on accurate and appropriate documentation
- Check vitals and perform physical examinations as appropriate (including monofilament and foot exams when applicable)
- Identify diagnoses for care management and active medical management in support of treatment
- Formulate a current and past medical conditions list using clinical knowledge and assessment findings
- Communicate assessment findings to inform the PCP of potential gaps in care
- Educate members on disease processes, medications, and compliance
- Initiate appropriate referrals
- Comply with HIPAA regulations and maintain PHI security
- Work autonomously with in-home visits when required and be comfortable with geriatric patients in clinical and nonclinical environments
- Active, unencumbered nursing license in the assigned state
- Active Nurse Practitioner Certification (ANCC, AANP, or AACN) as Adult, Geriatric, Family, or Acute Care NP
- Active or ability to obtain prescriptive authority in Louisiana post-hire
- 1+ year of clinical experience at the highest level of education
- Willingness to spend at least 1 hour with a member in their home; ability to travel to remote areas and environments with pets or tobacco users; may involve moderate risks or discomforts requiring safety precautions
- Ability to lift a 30-pound bag, navigate stairs, and adapt to various dwelling conditions
- Current BLS certification
- Valid driver’s license with reliable transportation to travel to member/patient sites
- Experience in Gerontology, Cardiology, Internal Medicine, Endocrinology, or Family Medicine
- Experience with electronic medical record systems
- Home care or home visit experience
- Computer literacy and strong administrative/organizational skills; ability to communicate effectively with seniors and families
Salary range is defined as total cash compensation varies based on experience, certifications, and location.
