Primary Care is evolving and changing both in the way in which we provide services to our patients as well as the way in which we work in practice. It is an exciting and dynamic time to work in General Practice/Primary Care with scope to develop both personally and professionally.

This is an amazing opportunity for a Registered Nurse to join our expanding multi-disciplinary team at Greyfriars Surgery. The post is 18 hours per week.

We are looking to recruit a Practice Nurse who will complement the current Practice Nursing and Medical Team to:

-

  • provide high quality care to patients within a primary and community care setting in accordance with CQC regulation, Practice Protocols and Policies.
  • have an interest in supporting people living with long term conditions (training will be provided as required)
  • develop the required skills, knowledge and experience in other specialist practice nursing areas as required

The successful candidate will be able to work both autonomously and as part of a wider multi-disciplinary primary care team.

The candidate must be able to commute within the Practices geographical area and have access to the appropriate means of transport.

Main duties of the job
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients
  • Diagnose and manage both acute and chronic conditions andensure the delivery of best practice for those registered patients, whilst developing and supporting General Practice Nursing duties
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Promote and deliver evidence-based care for patients presenting with aural conditions
  • Childhood and Adult immunisations and vaccinations including Influenza and COVID vaccines
  • National Screening Procedures and campaigns
  • Working collaberatively with the Practice Nurse team and wider PCN team

Job responsibilities

Job Purpose & Summary

The post holder is a qualified Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the Surgery. The post holder will demonstrate critical thinking in the clinical decision-making process.

They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and supporting the training of GP Registrars and other nursing staff as required.
