Position: Case Manager, Locksley Street

Case Manager – Locksley Street

Position at Pine Street Inn.

SCHEDULE: 25 hours per week, Monday‑Friday 3:00 pm‑8:00 pm.

PAY: $23.48 – $28.18 per hour DOE.

LOCATION: 7 Locksley St., Jamaica Plain, MA 02130 & other supportive housing sites. Position is 100% on‑site.

SUMMARY OF POSITION: The Supportive Housing Case Manager provides comprehensive, intensive case management support to homeless and chronically homeless individuals in housing using the Housing First Model. They work as part of an intensive team serving tenants with untreated mental illness, active substance use and other major health challenges, employing a Harm Reduction, Trauma‑Informed approach including Stages of Change Theory and Motivational Interviewing.

Responsibilities include managing a caseload, collaborating with Congregate Housing staff, assisting with program documentation, helping tenants obtain benefits, health care, education, employment, legal aid, developing daily living skills, creating and maintaining individual service plans and documentation.

QUALIFICATIONS Education and Training – Required
  • High School Diploma or GED
  • Strong written communication skills
  • Computer proficiency in Microsoft Office
Education and Training – Preferred
  • Undergraduate degree in social work, psychology, education, nursing or related field
  • Bi‑lingual – Spanish/English
Knowledge and Experience – Required
  • Experience working with individuals experiencing substance abuse, mental illness, trauma, homelessness or other complex disorders
  • Commitment to learning and using Harm Reduction and Motivational Interviewing skills
  • Knowledge of crisis intervention techniques and ability to respond appropriately
  • Experience working as part of a team
Knowledge and Experience – Preferred
  • Experience with chronically homeless individuals in Housing First programs
  • Knowledge of local caregiving and advocacy resources for substance abuse, mental illness, trauma, physical disabilities, relationship violence, criminal and civil justice involvement
  • Knowledge of government benefits systems and Boston area service providers
  • Experience with HMIS
