Job SummaryThe Clinical Pharmacy Specialist is responsible for all aspects of the medication use system in a designated clinical area. This includes daily monitoring, assessment and recommendations for pharmacotherapy, interdisciplinary patient care, and education for patients and team members. The specialist serves as the subject matter expert in the area, collaborating with pharmacy, medical, surgical, nursing, and other healthcare professionals to ensure appropriate pharmaceutical care.
Specialists will participate in the training and education of healthcare professionals, including pharmacists, technicians, nurses, and medical staff, and work independently under general supervision.
- Develop, implement, and maintain departmental policies and procedures related to safe and appropriate use of medications.
- Participate in daily patient care rounds in a designated practice area (cardiology, critical care, infectious disease, emergency medicine) and provide drug information and consultation with physicians, pharmacists, and nurses regarding drug therapy.
- Evaluate drug therapy for appropriate indications, dosage, drug interactions, and allergies; monitor patients’ regimens for effectiveness and adverse events, intervening as needed.
- Review the completeness and accuracy of medication profiles, reconciling and updating as required.
- Monitor prescribing and use of high-risk medications, ensuring safe and appropriate use.
- Act as a liaison between pharmacy, nursing, and medical staff to educate healthcare professionals on current drug-related procedures, policies, guidelines, and pathways.
- Identify adverse drug events and participate in the event reporting system, assisting in management, prevention, and process improvements.
- Participate in code blue responses related to designated areas.
- Provide patient education during and prior to discharge as appropriate.
- Identify opportunities for cost savings and optimal medication use to ensure appropriate patient selection for high-cost agents.
- Participate in multidisciplinary committees at the institutional and health-system level (Pharmacy & Therapeutics and practice-area specific committees as needed).
- Maintain an individual portfolio demonstrating specialist-level work for annual review.
- Serve as a preceptor to pharmacy students and PGY1/PGY2 residents in designated practice areas.
- Act on the Residency Advisory Committee.
- Participate in department activities aimed at professional development for all staff members including clinical pharmacists, technicians, residents, and students.
- Advise trainees and clinical pharmacists on projects such as medication use evaluation, research, continuing education, in-services, and lectures.
- Provide lectures for pharmacy, nursing, and medical staff as needed, focusing on practice-specific areas.
- Bachelor of Science in Pharmacy and a Doctor of Pharmacy (Pharm
D) with at least 2 years of hospital experience. - PGY-1 residency training with a minimum of 3 years of experience in the designated practice area.
- PGY-2 training in the designated practice area.
- Current registration as a pharmacist in Massachusetts.
- BLS/ACLS required.
- Board of Pharmaceutical Specialty certification in the specialty area, required within 1 year of hire if not already present.
- Ability to collaborate and effectively communicate with medical staff, pharmacists, nurses, and other health professionals.
