POSITION SUMMARY
Under the supervision of the director of specialty pharmacy and according to department policies, procedures, and guidelines, the Clinical Specialist will be responsible for providing the highest level of patient and customer care to patients undergoing treatment at the medical center. The Clinical Specialist is responsible for delivering comprehensive medication management to promote safe, effective, and cost‐conscious use of therapies. This role works collaboratively with physicians, nurses, pharmacy liaisons and other healthcare team members to design patient‑centered medication plans, monitor therapeutic outcomes, and provide evidence‑based clinical recommendations.
Additionally, the Specialty Clinic Pharmacist contributes to formulary stewardship, conducts medication‑use evaluations, supports quality improvement initiatives, and provides education to patients and healthcare staff, serving as the direct point of contact to provide clinical pharmacy support for all enrolled specialty clinics when needed.
Serves as a resource and drug expert for all general internal medicine related medications and strives to increase knowledge through research and writing activities.
ESSENTIAL RESPONSIBILITIES / DUTIESThe Specialty Clinic Pharmacist plays a key role in ensuring safe, effective, and accessible specialty medication therapy for patients with complex and chronic conditions through direct patient care as outlined in the scope of practice.
Specific responsibilities include, but are not limited to:
- Support timely access to specialty therapies by supporting prior authorizations, benefits investigations, and appeal processes.
- Provide disease‑specific and medication‑related education in accordance with departmental standards to support patient understanding, engagement, and adherence.
- Conduct comprehensive medication therapy reviews to assess safety, efficacy, and optimal use of specialty medications.
- Support enrollment and ongoing adherence monitoring for patients referred by care team and pharmacy liaison for Specialty Care Management.
- Collaborate with prescribers, nurses, case managers, and payers to ensure smooth initiation and continuity of therapy.
- Monitor clinical outcomes, adverse events, and adherence patterns to evaluate therapeutic outcomes and identify opportunities for intervention.
- Document all clinical activities, interventions, and outcomes to support quality improvement, regulatory compliance, and accreditation needs.
- Ensure all specialty pharmacy operations meet URAC/ACHC standards as well as manufacturer and payer requirements.
- Maintain accurate documentation and reporting to support audits and ongoing accreditation readiness.
- Contribute to the development of clinical protocols, therapeutic management pathways, and formulary strategies.
- Lead or support initiatives aimed at improving workflow efficiency, enhancing patient experience, and contributing to overall program growth.
- Provide in‑service education to pharmacy staff, students, nurses, physicians and other healthcare practitioners, as needed.
- Maintain own professional and specialized competency through continued education.
- Meet hospital‑wide standards in the following areas. Conform to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.

Job Requirements REQUIRED EDUCATION AND EXPERIENCE- BS in pharmacy or Pharm
D from an accredited college of pharmacy required. - Completion of PGY‑1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
- Specialty pharmacy experience.
- Completion of a PGY‑2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship is preferred.
- QS1 experience.
- Salesforce Health Cloud experience.
- Massachusetts pharmacist license in good standing by the hire date.
- Current Board Certification in Ambulatory Care Pharmacy (BCACP).
- Certified Specialty Pharmacy (CSP).
