Longwood Hospice’s Mission is to provide compassionate, holistic end-of-life care, centered on a personal approach, connectedness, dignity, and clinical excellence.
Focus of care is directed toward patient goals for the remainder of life and is adjusted and refocused as the condition of the patient changes.
The Hospice Aide provides compassionate, hands-on personal care to hospice patients in their homes or care facilities, under the supervision of a registered nurse. The aide plays a vital role in ensuring patient comfort, dignity, and quality of life during end-of-life care.

Essential Job Functions and Duties:- Assist patients with personal care, including bathing, grooming, dressing, toileting, and oral hygiene
- Help patients with mobility, transfers, positioning, and use of assistive devices
- Take and record vital signs as directed (temperature, pulse, respiration, blood pressure)
- Observe and report changes in patient condition to the Hospice nurse
- Provide skin care and help prevent pressure injuries
- Assist with light housekeeping related to patient care (changing linens, tidying patient area)
- Support patient comfort and emotional well-being with empathy and respect
- Follow the established hospice care plan and Longwood Hospice policies
- Maintain accurate documentation of care provided in Matrix Care
- Practice infection control and safety procedures at all times
- Respect patient privacy, confidentiality, and cultural preferences
- High school diploma or equivalent
- Completion of the MA Certified Nurse Aide, HHA or Hospice Aide training program
- Valid driver’s license and reliable transportation (for home hospice care)
- Compassionate, patient-centered approach to care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a Hospice care team
- Physical ability to assist with lifting, transfers, and extended standing
- Emotional resilience and professionalism in end-of-life situations
- Patient homes, hospice facilities, or long-term care settings
- May include evenings or weekends shifts
- Ability to lift and move patients with assistance or equipment
- Frequent bending, standing, and walking
