The role supports establishing care, coordinating referrals to primary care, withdrawal management, and therapy, while helping patients navigate insurance issues.
As part of Project ASSERT, you will document encounters in the EMR, screen for substance use, arrange outpatient visits, and collaborate with a multidisciplinary team.
