American Traveler is hiring an RN Care Coordinator for a Case Management role requiring an active MA RN license and Epic EMR experience.

• Work within the CCM Nursing unit coordinating care for an assigned caseload of patients
• Manage patients across the episode of illness from pre-admission through post-discharge
• Day shift schedule from 07:00 to 15:30
• 13-week assignment length
• Uses Epic for charting

• Active MA RN license required, no conditional or pending licenses accepted
• AHA-issued certifications only
• Flu vaccine required with no religious or medical exemptions allowed
• Proof of MA licensure required at time of consideration
• Two professional references required

• Coordinate interdisciplinary care plans utilizing patient assessments, care guidelines, protocols, and payer regulations
• Participate in ongoing evaluation of practice patterns to support quality, cost, and satisfaction outcomes
• Mobilize resources to maximize efficiency of care delivery
• Local candidates accepted if residing within 60 miles of the facility, at a reduced rate
• Report required time off (RTO) at time of consideration
• Timekeeping is managed through UKG/Kronos
