We're hiring at Compass Adult Foster Care!
Compass Adult Foster Care helps people in Massachusetts get the daily care they need without leaving home. Through the Mass Health Adult Foster Care (AFC) program, members live in their own home or with their caregiver. Their caregiver is often a family member, friend or neighbor who is already helping them, and through Compass AFC that caregiver is trained and paid for the care they give.
Every household gets ongoing support from a dedicated nurse and case manager.
Visit members at home, review medication and health status, write and update clinical documentation, and coordinate with prescribers and case managers. You carry a caseload rather than a shift.
WHAT WE ASK FOR- Active Massachusetts RN or LPN license in good standing
- Comfort working independently in members' homes
- Reliable transportation and a valid driver's license
- A second language is valued
Field-based in Middlesex, Suffolk, Worcester, Essex, Plymouth and Bristol counties
