Requirements for Licensed Social Worker:Must possess a B.A in Social Work, LSW or equivalent degree AND a current social work license.
LSW, LCSW, or LICSW required.
1+ years skilled nursing facility experience preferred.
Compiles social history data.
Participates in assessments, development and evolution of resident's care plan.
Responsible for planning and leadership interdisciplinary care planning weekly meetings.
Documents timely progress notes according to state and federal regulations.
Coordinates the discharge planning for residents.
Able to interpret and implement established social service policies, procedures and regulations.
Coordinates services of community agencies for continued delivery of care upon discharge.
