Care Manager MSW and Care Manager LCSW . If you meet the qualifications for either position, we encourage you to apply!

As a Clinical Social Worker, you will:

• Partner with patients, families, and healthcare teams to advocate for social and emotional needs.
• Provide a full range of social work services, including psychosocial assessments and treatment planning.
• Facilitate discharge planning and crisis intervention.
• Collaborate with interdisciplinary teams to enhance patient care and resource utilization.
• Lead efforts to achieve patient goals and desired clinical outcomes.

• Master's Degree in Social Work from an accredited institution.
• Washington Clinical Independent Social Worker Associate License (for MSW) or Clinical Independent Social Worker License (for LCSW) upon hire.
• At least 1 year of experience in acute medical or mental healthcare settings.

Care Manager MSW: $45.66 - $69.47

Care Manager LCSW: $48.39 - $73.64

• ACM/CCM certification.
