In-Home Health Clinician - Nurse Practitioner or Physician Assistant (Full Time) – Grand Junction, CO

Base pay range: $87,035.00/yr - $/yr

This role involves conducting In-Home Health Evaluations for members in their own homes, engaging with people to bridge gaps in care, and delivering flexible, comprehensive, and personalized health evaluations.

You will provide in-home health evaluations as a Full Time Clinician (Nurse Practitioner or Physician Assistant). Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the member’s health plan, additional diagnostic tests. You will use an iPad and a clinical workflow for each evaluation. You are not required to prescribe medicine, order labs, or alter current treatment regimens.

• Provide Diagnostic & Preventive Services such as spirometry, eye exams, bone density screening, and related services

Primary workplace is the member’s private residence. Environments may vary and can include household pets, secondhand smoke, and other potential airborne irritants. Candidates must be able to perform duties safely in homes with diverse conditions.

• Active, unrestricted license(s) in the coverage area(s)
