Are you passionate about making a difference in people's lives? Sol Recovery Community, Inc. is seeking a dedicated Case Manager to join our team! This role is essential in supporting clients on their journey to recovery by connecting them with resources, coordinating referrals, and ensuring a seamless intake process. Additionally, this position includes on-site case management services through our supported living program.
Key Responsibilities- Guide clients and families through the intake and referral process.
- Ensure completion of intake documentation.
- Develop and maintain relationships with community partners.
- Meet with clients individually to assess and address their needs.
- Provide on-site case management services for supported living clients.
- Connect clients to vital community resources and higher levels of care.
- Coordinate referrals to residential and detox facilities.
- Observe and document urinalysis testing as required.
- Maintain detailed case management notes in compliance with SRC policies.
- Collaborate with the clinical treatment team and attend weekly case staff meetings.
- Uphold confidentiality and ethical standards.
- Assist with community outreach and presentations.
- Perform other job-related duties as assigned.
- Must be at least 18 years of age.
- Social Service Worker license (SSW) or Case Management Certification (or willingness to obtain).
- If not already certified, the case manager will be supported in completing Domestic Violence (DV) Advocacy Training to ensure they are equipped to work with this population.
- Clean criminal record (no felonies or related misdemeanors per licensing standards).
- Must complete a TB screening and test only if indicated.
- Ability to pass post-offer and random drug screenings.
- Strong computer skills, including Google Workspace, data management, and email.
- Reliable and punctual with excellent communication skills.
- Ability to read, write, and communicate fluently in English.
- Primarily an office-based role with some on-site case management responsibilities.
- Requires the ability to sit for extended periods, communicate effectively, and visually observe UA testing.

Starting Salary DOE
Salt Lake City, UT $46,353.60-$46,535.60
