Prime Healthcare is seeking a dedicated and experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Family Practice team in Bourbonnais, Illinois
. This is a full-time opportunity to provide high-quality, patient-centered primary care in a community-focused outpatient clinic.

• Full-time, outpatient primary care role
• Provide care across the lifespan, focusing on prevention, chronic disease management, and acute concerns
• Collaborate with physicians and support staff in a team-based care model
• Build meaningful relationships with patients in a supportive community setting

• Licensed Nurse Practitioner or Physician Assistant in the state of Illinois
• Board certification in Family Medicine (NP) or relevant PA specialty
• Excellent communication and clinical decision-making skills
• Passion for delivering high‑quality, compassionate care
