Licensed Practical Nurse

Overview: This is a direct service position providing individual medical support and education to individuals in assigned programs. It involves advocating for all medical needs of program participants, focusing on optimal health for all individuals. Your role as a Licensed Practical Nurse (LPN) includes providing basic medical care across a broad spectrum of needs while maintaining quality, service excellence, and professional standards.

Hours are assigned by a supervisor with on‑call responsibilities.

Pay: $41.00 per Hour

Responsibilities
  • Assess and coordinate healthcare services for program participants in collaboration with clinical and residential teams.
  • Communicate medical updates and follow‑up needs to relevant staff, including the RN, Residential Manager, and Health Care Director.
  • Accompany and assist individuals with medical appointments as required by care plans and DDS standards.
  • Document all nursing interventions, health changes, and communications in TIER and other designated systems.
  • Review and support implementation of Individual Support Plans (ISPs), including self‑medication, hygiene, and therapy routines.
  • Maintain and monitor medication and treatment orders; report side effects or concerns to appropriate parties.
  • Train staff on medication administration policies and ensure adherence to protocols.
  • Keep health records current and compliant with DDS regulations.
  • Share relevant health information with residential and day program staff to ensure continuity of care.
  • Participate in staff meetings, supervision, and mandatory in‑service trainings.
  • Promote safety, dignity, and respect for all individuals supported.
  • Uphold confidentiality and professional standards in all interactions.
  • Collaborate effectively with the care team and contribute to a positive, team‑based environment.
  • Perform additional duties or medical interventions as assigned by nursing leadership.
Qualifications
  • LPN or RN with current MA license.
  • Associate’s degree required.
  • 1 – 3 years of related experience.
  • Knowledge of medication administration procedures and familiarity with the Medication Administration Program (MAP) preferred.
Skills and Knowledge
  • Valid US Driver’s License and clean driving record required.
  • Ability to assess, plan, implement, and evaluate patient care in accordance with scope of practice.
  • Strong clinical documentation skills and familiarity with Electronic Health Records (EHR) systems.
  • Understanding of and ability to comply with HIPAA, DDS regulations, and healthcare standards.
  • Strong interpersonal skills with the ability to communicate effectively with individuals, families, and interdisciplinary teams.
  • Demonstrated ability to handle medical emergencies and follow safety protocols.
  • CPR and First Aid certification (or ability to obtain upon hire).
  • Commitment to providing person‑centered, respectful, and inclusive care.
