What you will do

  • Conduct comprehensive assessments under the Care Act 2014 to support young people with Education Health and Care Plans (EHCPs) as they transition to adulthood.
  • Develop and implement personalised support plans that focus on strengths-based and person-centred approaches.
  • Manage complex caseloads, exercising professional judgment and adhering to relevant legal and policy frameworks.
  • Collaborate with multi-agency partners from education, health, and other sectors to ensure cohesive support during the transition process.
  • Engage with young people and their families, providing expert advice and clear communication throughout.
  • Utilise case management systems like Mosaic to record assessments and care plans accurately.
  • Produce high-quality analytical written work, including court reports and legal proceedings documentation if necessary.
  • Apply a robust understanding of the Mental Capacity Act 2005, completing mental capacity assessments and best interest decision-making frameworks as needed.
