About the Position:

During visits lasting up to one hour, Matrix providers review health, medical history, medications, social environment, and other risks, providing insights into overall health that routine visits may miss. The Comprehensive Health Assessment aims to improve care quality and close care gaps efficiently.

Responsibilities:

Matrix Provider Expectations:

• Conduct assessments, medical history, diagnosis, treatment, education, referrals, follow-up, and documentation per guidelines
• Collaborate with physicians, case managers, social workers, families, and medical personnel
• Coordinate with Primary Care Physician on patient education and follow-up
• Provide services via home visits, skilled facility visits, and telehealth

Nurse Practitioner - PRN - Home Risk Assessments

Type: PRN/Part Time, flexible

Pay: Earn $1,800 to $9,000/month (based on 20-100 visits at $90 each)

Location: Home, SNF, Telehealth, community in Smiths Grove, KY

Hours: Days, evenings, nights, weekends, flexible

Benefits include 401(k) and limited voluntary benefits; non-benefited providers eligible for mileage reimbursement.

Qualifications:

• Master’s Degree or equivalent experience with NP licensure
• Current RN and NP licensure with prescriptive authority
• Board certification (AANP, ANCC, AACCN) in approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum 1-year NP experience
• Strong computer and record-keeping skills
• Excellent communication skills
• Flexibility with change

Travel Requirements:

• Valid driver’s license, automobile insurance, ability to travel to patient locations
