LSW / LPC: $55,000 – $68,000 annually

Position Summary:
Unison Health is seeking a Clinical Therapist to join our Assertive Community Treatment (ACT) team, providing direct mental health services to clients with severely persistent mental illness (SPMI). This position involves delivering individual, group, and family therapy, conducting diagnostic assessments, and providing advocacy and linkage services under the supervision of an independently licensed clinician. This role is ideal for clinicians who are committed to assertive community-based care and enjoy working collaboratively within a multidisciplinary team to support clients with complex behavioral health needs.
Key Responsibilities & Role Highlights:
Provide community-based mental health services for clients with SPMI
Work collaboratively as part of a ACT team with flexible schedule including evenings, weekends, and holidays
Opportunities to engage in diagnostic assessment, individual, group, and family therapy
Participation in team meetings, supervision, in-service training, and professional development
Emphasis on advocacy, care coordination, and client-centered treatment
Provide diagnostic and therapeutic services to clients under the supervision of an independently licensed clinician
Deliver assertive community treatment (ACT) services according to state mandates and agency/program policies
Document all services accurately in the electronic health record system
Maintain client confidentiality and ensure proper handling of release of information
Participate in team meetings, supervision, training, and continuing education to enhance knowledge and promote coordinated care
Education & Experience Requirements:
Bachelor’s degree in Social Work (Master’s preferred) or Master’s in Counseling
Minimum one year of experience working with clients with SPMI
LSW or LPC licensure required
Knowledge of mental illness, crisis intervention, assessment, and counseling techniques
Understanding of chronic physical health issues common among SPMI or SED populations
Ability to analyze problems, interpret data, and make informed clinical recommendations
Ability to work a flexible schedule and participate in on-call rotations
