• Perform evaluations and develop treatment plans.
• Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for safety, and provide referral and education resources.
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members.
• Supervise Occupational Therapy Assistants as needed.
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.

• Minimum of a bachelor’s degree in Occupational Therapy from an accredited OT program.
• Current, unrestricted license as an Occupational Therapist in state of practice.
• Current Live CPR Certification.
• Two‑step TB skin test required.
• Proof of Auto Insurance.
• Demonstrated knowledge of evidence‑based therapy interventions.
• Familiarity with assisted living, independent living, geriatrics and/or home health.
• Effective verbal and written communication skills.
• Ability to lift 50 pounds.
• Ability to lift up to 100 pounds following safety procedures.
• Fine motor skills, visual acuity, walking/standing extended periods.
• Physical requirements:
  
  pushing/pulling, bending, stooping, reaching, kneeling, frequent positioning.
• Health screen may be required before assignment and periodically thereafter to verify capability to perform duties.

$50.00 - $55.00 / Hour
