Description

The purpose of the RN Case Manager position is to support the physician and interdisciplinary team in facilitating patient care with the goal of enhancing clinical outcomes and patient satisfaction while managing the cost of care. The RN Case Manager coordinates care facilitation and discharge planning in partnership with the UR RN and Social Work Case Manager, maintains a designated patient caseload, plans effectively to meet patient needs, manages length of stay, and promotes efficient resource utilization.

Key responsibilities include:

  • Facilitate collaborative management of patient care across the continuum, intervening to remove barriers to timely and efficient care delivery and reimbursement.
  • Apply process improvement methodologies to evaluate outcomes of care.
  • Coordinate communication with the healthcare team, patients, families and post‑acute care services.
  • Screen for and prevent readmissions.

Estimated salary range: $73,860.80 – $96,019.04 per year, depending on experience.

Qualifications
  • Associate’s degree.
Licenses & Certifications
  • Registered Nurse.
Additional Qualifications
  • RNs hired prior to February 2012 (10 / 1 / 2017 at Bethesda or 7 / 1 / 2019 at BRRH) with an Associate Degree are not required to have a BSN to continue their non‑leadership role as an RN; however, they must complete a BSN within 5 years of their job entry date.
  • Minimum of 3 years hospital clinical experience, with at least 1‑3 years hospital or payer Case Management experience preferred.
  • Excellent interpersonal communication and negotiation skills.
  • Strong analytical, data management and computer skills.
  • Current working knowledge of discharge planning, utilization management, case management and performance improvement preferred.
  • Understanding of pre‑acute and post‑acute venues of care as well as post‑community resources preferred.
  • Strong organizational and time‑management skills, evidenced by the capacity to prioritize multiple tasks and role components.
  • Ability to work independently and exercise sound judgment in interactions with the health‑care team, patients and families.

Minimum required experience: 3 years.
