- Hospital-based clinic
- Patient census: 11 patients/day (no pediatrics)
- Cases:
Chronic disease management (diabetes, hypertension), medication refills, occasional cold/flu - Support: APP and MD backup, full hospital subspecialty support
- EMR:
Epic - No procedures required
- Board Certified in Internal Medicine (Family Medicine not accepted due to bylaws)
- Montana Medical License (or willing to license)
- DEA registration required
- BLS certification required
- 13 physicians and 10 APCs across 3 locations
- Geriatric, palliative care, and home care teams
- NCQA Level 3 Patient Centered Medical Home
- CPC+ participant
- Active educational site for NP, PA, BSN, MA students
- ACP educational opportunities available
