The Pennant Group is seeking a Mobile Advanced Practice Clinician (APC) to support our 24/7 mobile clinic operations. The APC will provide comprehensive care, including history taking, assessment, diagnosis, prescriptions and education, while maintaining documentation and confidentiality.
Under Medical Director supervision, you will drive clinical quality across ALF, SNF and home visits, and collaborate with care teams to ensure timely, cost-effective services within scope of practice.
