Required Qualifications:

• Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.)
• Board Certified or Board Eligible in Internal Medicine, Family Medicine and/or Geriatric Medicine
• Valid, unrestricted Florida Medical License (or able to obtain)
