Aviata Health Group is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) to join our team in a Skilled Nursing and/or Assisted Living setting focused on geriatric care.

Location: Aviata Bradenton, FL

Base pay range: $70,000.00/yr - $80,000.00/yr

Core responsibilities:

• Provide psychotherapy services to the geriatric population within a skilled nursing facility.
• Collaborate with interdisciplinary team members (Nurse practitioners, psychiatric nurse practitioners, psychologists, LCSWs, and podiatrists) to develop and implement individualized care plans.
• Deliver direct counseling and crisis intervention using evidence‑based practices.
• Advocate for patients and families, ensuring culturally competent care.

Qualifications:

• Hold an active and unrestricted LCSW license in the state of Florida.
• Completed a Master’s degree in Social Work (MSW).
