Overview

The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in Chen Med/Jencare medical centers, nursing homes, skilled nursing facilities (SNF) and home settings depending on the assignment. Responsibilities include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow-up and documentation according to Chen Med standards for quality, service, productivity and teamwork.

Participation in clinical rounds and conferences and thorough documentation through progress notes and summaries are also included. The NP must work independently and in collaboration with other health care professionals, with consults to managers, physicians, and the medical director to ensure guideline compliance. This role may involve risk and quality management programs and other meetings. The NP will adhere to departmental goals, standards of performance, regulatory compliance, quality patient care, and policies and procedures.

The NP practices as a certified nurse practitioner for a patient population in collaboration with a licensed patient care team physician and in accordance with a written or electronic practice agreement.

Job Duties / Responsibilities
  • Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops and implements a patient management plan. Provides patient/family counseling and education.
  • Plans patient care based on in-depth knowledge of the patient population and protocol, anticipating and identifying physiologic or psychological problems, considering cultural background and support systems. Serves as patient advocate.
  • Patient management includes:
  • Writes admission, transfers and discharges orders.
  • Orders and interprets laboratory and diagnostic studies.
  • Orders and administers medications and treatments as appropriate.
  • Refers patients for consultations when indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, ICU, infectious disease, hematology, psychiatry, social service, dietary, etc.).
  • Documents through in-depth progress notes and summaries.
  • May perform invasive procedures independently upon documented competency.
  • Participates in patient care rounds and conferences, communicates management strategies to the patient care team, and collaborates with the multidisciplinary team.
  • Recognizes situations requiring immediate physician attention and initiates life-saving procedures when necessary.
  • Uses advanced communication skills to solve complex problems and improve patient care and service.
Other Responsibilities May Include
  • Collaborates with multidisciplinary team members to analyze and improve health care delivery systems and practice patterns.
  • Participates in outside activities that enhance personal and professional growth.
  • Initiates arrangements and writes orders for SNF discharges and completes appropriate paperwork.
  • Works with physicians, nurses, PTs, social workers, families, and key caregivers to transition the patient to a lower level of care as medically appropriate.
  • Introduces self to patient/family and explains the nurse practitioner role.
  • Facilitates patient/family conferences to review treatment goals, optimize resources, provide education, and identify post-SNF needs.
  • Supports informed decision-making regarding goals of care, palliative care, and hospice.
  • Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.
  • Presents prescriptions in accordance with state practice.
  • Other duties as assigned and modified at manager’s discretion.
