• Act as a Skilled Nursing Facility’s leading provider of internal medicine services.
• Collaborate with the facility’s interdisciplinary team and physicians, serving as a day‑to‑day clinical leader while overseeing the care of all residents.
• Work with the entire patient population—short and long‑term residents—oversight the day‑to‑day care with nursing staff and medical directors.
• Evaluation of new admission visits.
• Coordination of discharge visits.
• Monthly stability visits for long‑term residents.
• Evaluations for patients exhibiting changes in condition.
• Provide advanced care planning.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in direct management of acute and chronic illness and disease.
• Prescribe all necessary medications and treatments.
• Perform comprehensive physical examinations of patients.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Communicate with attending physicians and/or specialists regarding patient care.

• Advanced Practice Licensure (APN) with a Florida license.
• Must have an active license and DEA number.
• Previous experience in nursing or other medical fields.
• Ability to build rapport with patients and their loved ones.
• Ability to thrive in a fast‑paced environment.
• Excellent written and verbal communication skills.
• Strong leadership experience with emphasis on teamwork in a healthcare setting.
• Master’s degree required.
