Navigation Specialist - Hybrid

Mind Care Solutions is seeking a dependable, tech-savvy, and compassionate Care Navigation Specialist to support psychiatric telehealth services and Behavioral Health Integration initiatives in Post-Acute Care settings. As a Care Navigation Specialist, you will work alongside a remote Psychiatrist or Psychiatric Mental Health Nurse Practitioner (PMHNP) to facilitate smooth communication and clinical workflow between the Provider, patients, and facility staff. This role is ideal for nursing students, medical assistants, or healthcare professionals seeking flexible, part-time work with exposure to psychiatric and geriatric care.

$17.00 per hour

up to 25 hours per week, dayshift

hybrid, Warren County and surrounding area

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Serve as the on-site liaison between the telehealth Provider and facility staff during psychiatric evaluations.
  • Operate and troubleshoot telehealth equipment to ensure high-quality virtual visits.
  • Communicate effectively with patients, many of whom may have psychiatric illnesses, cognitive impairments (e.g., dementia), and complex medical comorbidities.
  • Escort or assist patients to designated telehealth areas and prepare them for virtual appointments.
  • Clearly relay questions and responses between patients, providers, and facility staff.
  • Document key visit information as directed by the Provider or Mind Care team.
  • Support up to 20 patient visits per 4-hour shift, potentially working with more than one provider during a shift.
  • Coordinate with facility staff to ensure timely transitions between patient visits.
  • Support Behavioral Health Integration (BHI) services by evaluating clinical events, monitoring patient follow-up needs, and collecting relevant information to support continuity of care.
  • Review and assess documentation from facility records to help identify psychiatric and behavioral health needs.
  • Communicate and document notable clinical observations, patient concerns, or changes in condition for Provider review.
  • Travel to multiple facilities as scheduled; mileage reimbursement provided.
  • All Other Duties as Assigned:
    Perform additional responsibilities as needed to support the organization’s mission and goals.
REQUIRED QUALIFICATIONS
  • High school diploma or equivalent required.
  • Enrollment in or completion of a nursing, medical assistant, or related clinical program preferred.
  • No specific licensure required for the role.
  • Minimum of 1 year experience in a healthcare or patient-facing environment (e.g., CNA, MA, EMT, nursing student).
  • Demonstrated ability to interact professionally with clinical staff, patients, and providers.
KEY SKILLS & CERTIFICATIONS
  • Basic medical knowledge and comfort working with patients experiencing psychiatric illness, cognitive impairment, and/or medical comorbidities.
  • Strong interpersonal and communication skills.
  • Technological proficiency, including ability to operate telehealth devices (tablets, laptops, AV equipment).
  • Reliable transportation for travel between facilities (mileage reimbursement provided).
PREFERRED QUALIFICATIONS
  • Experience in telehealth, virtual care coordination, or behavioral health settings.
  • Familiarity with HIPAA regulations, electronic health records (EHR), and clinical documentation practices.
  • Prior work in post-acute, long-term care, or geriatric settings a plus.
  • Ability to support high-volume workflows (up to 20 patients per 4-hour shift).
