Be part of a dynamic team to provide strength-based community case management services to individuals who possess a mental health diagnosis. Duties include development of recovery-oriented treatment plans, collaboration with assigned treatment team, link with community-based resources and services, provide advocacy and support individual recovery journeys, and coordinate with crisis services through an on-call rotation.
