The WISHH Primary Care Network (PCN) is seeking an enthusiastic Advanced Care Practitioner, paramedic, or newly qualified GP or tojoin our PCN as part of the Additional Roles Reimbursement Scheme (ARRS). Thisis an exciting opportunity to be part of an innovative team focused on delivering patient-centred care in a collaborative and supportive environment.
You will have the unique opportunity to work across our PCN,which covers 2 practices (Idle Medical Centre and The Saltaire and Windhill Medical Partnership) in the North Bradford District (close to the Leeds Border). This is a fantastic opportunity to experience the diversity of primary care within the WISHH PCN, working on a full or part time basis.
Salary will be commensurate with experience, and in linewith the guidance set out in the NHS England Network Contract DES Contract Specification 2025/26.
The successful application will join a dedicated Housebound Visiting Team. One member of our team also undertakes a weekly care home wardround, thus supporting some of our most frail and medically complex patients.
Main duties of the job- Primarily, visiting patients at home and making a clinical assessment of their acute and/or chronic medical problems. Actioning further management accordingly (arranging bloods or other investigations, referrals, prescribing as indicated, or admission if necessary).
- Other clinical duties may occasionally include face to face consultations within the surgery, telephone consultations and queries, visiting patients at home, prescription administration and management, general administration (referrals, pathology, letters/communications, correspondence) in a timely fashion, ensuring high quality care.
- Making decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation.
- Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems.
- Screening patients for disease risk factors and early signs of illness.
- Developing care plans for health in consultation with patients and in line with current practice disease management protocols.
- Providing health education to patients.
- Contribute to optimise PCN Investment and Impact Fund (quality improvement markers)
- Recording clear consultation notes to general medical practice standards.
- Collecting data for audit purposes and complying with QOF requirements.
- Compiling and issuing computer-generated acute and repeat prescriptions.
- Prescribing according to local and national guidelines.
Our PCN includes two practices, comprising Idle Medical Practice and the Saltaire and Windhill Medical Partnership. Our total PCNpatient population is approximately 37000, with Saltaire and Windhill making uptwo thirds and Idle one third of this cohort.
We are a forward thinking and innovative PCN and embrace the challenges of the ever-changing primary care landscape. With the use of effective IT systems, collaborative working, and a strong focus on education and learning, we deliver safe and high-quality care for our patients. We offera number of specialist services in-house, including dermatology, diabetes, MSKas well as others, and further opportunities are available for clinical support and development.
We are a friendly and supportive team, and tailored mentoring and clinical supervision will be provided for the successful applicant.
Our PCN Team itself comprises a Clinical Director (GPpartner); PCN Manager; two ACPs; two care coordinators and an administrator.
Regular team meetings take place including a weekly PACT meeting, and a monthly team education and support meeting. Additionally, monthly education taking place within both practices, as well as provision for annual mandatory training, are available.
OurPCN was one of the very first to develop a dedicated PACTeam (Proactive Care Team), undertaking proactive reviews of housebound patients and interfacing with other professionals such as physiotherapists, occupational therapists andsocial prescribers.

In accordance with the practice rota, as agreed,the post-holder will be available to undertake a variety of duties comprising mainly visiting patients at home, but may include surgery consultations,telephone consultations and queries, care home ward rounds, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
