Position: Support Worker (REACH)

LOCATION:
Community based across the Bradford District

GRADE / SALARY:
D1 £ 27,294.18 FTE per annum

HOURS:
37.5 hours per week

Purpose of the Job:

To work as part of the Reach team and alongside the Community Mental Health Teams (CMHTs) to support clients with Severe mental illness (SMI) to reach their goals.

Main Duties
  • Hold a caseload of clients, providing one to one support for individuals in a recovery-focused and person-centred way, which builds confidence and helps people to move forward.
  • Use your own lived experience of recovery from mental health problems, as appropriate, to inspire and support others.
  • Ensure that client notes are recorded in an accurate and timely manner and kept updated.
  • Work in line with all relevant safeguarding policies and procedures, ensuring concerns are escalated appropriately.
  • Work closely with other health and social care professionals including statutory and voluntary agencies to ensure that support is joined up and of high quality.

Method

Educated to GCSE Level 4 or above, or equivalent experience

Awareness or knowledge and/or lived experience of the challenges facing people who have difficulties relating to their mental health.

Experience

At least 2 years’ experience of working one to one with clients who face challenges with their mental health, including with clients who have SMI.

Some experience of working in a multi-agency health or social care setting

Experience of using IT systems to record client notes
