Bradford District and Craven Mind is recruiting a Health Engagement Practitioner to support people living with Severe Mental Illness through our PHC service. You will work across GP practices and in home visits with Health Engagement Officers, delivering person-centred care to increase participation in annual checks and other interventions.
This hybrid role requires compassion, strong communication, and ability to collaborate with local care networks.
