Assess needs and implement (and review) care packages that meet the Care Act 2014
Work with individuals and their families to support people to reach their full potential through self care and enablement
Complete and review enablement support plans
Supervise and support a team of enablement support workers to ensure the BEST service is delivered in accordance will all relevant departmental polices and guidelines to enable compliance with the care quality commission (CQC).Carry out performance managements including supervisions, appraisals and observations.
Plan and lead team meetings.
Support and mentor trainee enablement support workers to achieve the care certificate and level 2 in health and social care Participate in flexible and integrated working with other partners.
QualificationQAF diploma or Level 2 in Health and Social Care and a willingness to achieve Level 3
Driving LicenceA full UK driving licence
Experience
Significant (and demonstratable) experience of working in adult care Hours of work
37 hours per week over 5 days Shifts between the hours of 7am - 10pmIncludes weekend and bank holiday working.
