Mersey and West Lancashire Teaching Hospitals NHS Trust is seeking an experienced Advanced Clinical Practitioner to lead dementia and delirium care at Whiston Hospital. The role is a permanent, full-time position under Annex 21 AfC Terms and Conditions, 37.5 hours per week.
You will provide autonomous, high-quality, person-centred care and lead assessment, management and education across multidisciplinary teams to improve outcomes and reduce admissions.
