About the Role:
The Health and Social Care Committee is looking for an experienced and compassionate leader to join the senior leadership team as an Associate Director of Community Health and Social Care. This is a pivotal role, providing strategic leadership across community-based health and social care services, driving innovation, service improvement, and integrated working to deliver excellent outcomes for the community we serve.
The successful candidate will be passionate about person-centred care and committed to shaping sustainable services for the future.
This is a key leadership role within HSC, operating as part of the senior leadership team and enabling the Director of Care Delivery to fulfil their role in leading high-quality services that deliver the best outcomes for those individuals within Community Health and Social Care across the Bailiwick of Guernsey.
The Associate Director provides leadership, operational management, and strategic direction of the service area in line with HSC Care Values. They will lead services that deliver high quality outcomes against best practice and professional standards and demonstrate value for money. They will promote a culture of openness and transparency whilst enabling their teams to engage in person centred service developments through the annual service planning cycle.
This postholder should have experience of transforming care and implementing long term strategy. They will have extensive experience within Community Health and Social Care settings with the ability to motivate and inspire staff to deliver the highest possible clinical and professional standards. The postholder will be a member of the Professional Alliance of Health and Social Care Practitioners.
The post holder provides leadership in respect of the key professional areas for practitioners within service level teams and will focus on the following key areas:
- Community Nursing
- Extra Care Housing
- Social Care – Home Help
- Palliative Care
- Adult Physical Disability
- Frailty Team
- Adult Safeguarding
- Rapid Response and Reablement
- Adult Social Work
- Respite and Sitting Service
- Social Care – Community Carers
- Tissue Viability
