Advanced Nurse Practitioner

Overview

Horton Park Medical Practice is looking to appoint a motivated, enthusiastic, experienced Practitioner to join our friendly clinical team.

The Advanced Nurse Practitioner/Advanced Care Practitioner will work autonomously within their professional scope of practice, providing acute care for patients via face-to-face, video and telephone consultations and, when required, home visits. They will take initial histories, carry out clinical assessments, make diagnoses, provide appropriate treatment and evaluate care, and make referrals for investigations in a timely manner.

The post holder will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will work collaboratively within the practice team to meet patient needs and support the delivery of policies and procedures. They will develop a leadership role within an area of clinical practice.

Experience
Essential

• Experience of working in a community setting
• Experience of managing minor ailments

Desirable

• Experience of working in a GP practice
• General Nursing experience across various chronic diseases
• Palliative care experience

Qualifications
Essential

• Recognised Advanced Nurse Qualification (MSc)
• Post registration experience

Desirable

• Primary Care Experience
• Nurse led management of minor illness, minor ailments and injuries
• Nurse led triage
• Diabetes Diploma
• Respiratory Diploma
• Cytology
