Care Assessor

We are currently working with a Local Authority client in the Northwest London area who is looking to recruit an experienced Care Assessor to join their Learning Disabilities Support Planning Team . In this role you'll:
Assess adults with learning disabilities and complete person-centred support plans. Manage, review, and monitor care packages, ensuring services meet individual needs. Work closely with families, carers, health professionals, and partner agencies to achieve positive outcomes. Support safeguarding processes and contribute to multi-disciplinary assessments and reviews. Essential experience and skills:
Experience completing social care assessments and support plans. Knowledge of learning disabilities, safeguarding, and adult social care legislation. Experience working with vulnerable adults within a local authority, NHS, or similar setting. Strong communication skills and the ability to work effectively with partner agencies and families. Contract Length : 3-6 months with the view to extend Pay Rate : £22.41 PAYE Working pattern - 37 hours per week, Hybrid working arrangements.
