Physical Therapist; PT - Home Health

Champion Rehab is looking for a compassionate, innovative and energetic Physical Therapist to join our award winning therapy team expanding to Brainerd, MN in 2026. Check out Pinecrest of Country Manor!

This position offers the unique opportunity to gain experience and knowledge within an inpatient, outpatient and homecare setting.

• Provide direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, on‑going modifications to existing plan of care as needed, and ability to initiate appropriate referrals to other disciplines when needed
• Work closely with a skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities
• Apply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Physical Therapy Assistant, and communicating with physicians to ensure fluency of care
• Complete documentation which follows regulatory guidelines (state, federal)

• Graduate of an accredited Physical Therapy program
• Currently licensed to practice in the State of MN. New graduates and temporary license holders are encouraged to apply
