Epic Nursing Services is seeking a full time Program Care Manager to assess the needs, initiate and coordinate services for frail elders and disabled adults enrolled in agency programs. The Care Manager conducts in-home visits, participates in an inter-disciplinary team and works collaboratively with community agencies.

Qualifications
  • ABachelor’s or Masters Degree in social work, human services or a related field required.
  • One-year elder care and disabled adults case management experience preferred.
  • Excellent computer skills required, including thorough knowledge of Microsoft Word, Outlook and Excel. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential.
  • Excellent assessment and documentation skills required.
  • Ability to work independently and be self-motivated.
  • Spanish and Mandarin speaking candidates are encouraged to apply.
  • Must have MA drivers license the agency will provide transportation to conduct home visits.
  • Responsibilities
  • Assist elderly and disabled consumers in developing and maintaining maximum independent living within the community
  • Responsible for assessing the needs of program participants and to develop, monitor and review service plans for all clients
  • Position requires home visits to elders, coordinating service plans, monitoring clients progress, corresponding paperwork, data entry and statistics.
  • Obtaining a social history and conducting a psychosocial evaluation.
  • completing a care-management progress note corresponding with each on-site visit or encounter and upon significant change.
  • Reviewing direct care aide logs daily or at a minimum weekly
  • reviewing and documenting the fire and safety procedures for the qualified setting; participating in discharge and transition planning and implementation.
  • Evaluating, supporting, and training direct care aides.
  • Providing timely assistance and responding to urgent or emergency needs of the member
  • Assists in developing an outreach plan, identifying priorities and a reasonable timetables
