Licensed Clinical Social Worker Long Term Care

Pinnacle Group of Hudson Valley Ltd

Social Worker reports to the Director of Social Services and ensures that assessments and care plans are developed in a timely and meaningful manner.

Salary

Base pay range: $35.00/hr - $40.00/hr (actual pay based on skills and experience)

Qualifications
  • Bachelor’s or master’s degree in social work
  • Valid and current LCSW or LMSW license
  • Clinical social work experience working with residents with disabilities (TBI, Behavioral Health) is preferred
  • Excellent listening and communication skills
  • Ability to make psychosocial assessments and develop and implement care plans
  • Ability to provide after-hours service as needed
  • Prior experience working in a Social Service department in a Skilled Nursing Facility for 2 years or more is a plus
  • Knowledge of community resources including VNA, Elder Services, Adult Day Programs, Assisted Living Communities, Rest Homes, Senior Housing, etc.
Responsibilities
  • Provide residents and families with education and support regarding aging, illness, coping and loss.
  • Demonstrate an understanding of advanced directives, healthcare proxies, guardianship requirements and the like.
  • Act as team leader in discharge planning and coordination of services needed for a successful transition back to the community setting.
  • Evaluate patients (existing, new and prospective) based on their needs, limitations and desires.
  • Address patient concerns and goals while maintaining constant communication with the patient and case manager.
  • Build rapport with patients and their families.
  • Coordinate care with PT and OT departments.
  • Develop care plans for patients.
