Position: Outreach Case Worker (Full‑time)

Schedule: Sun–Thurs. Hours are a combination of 8 a.m.–4 p.m. (‑3 days) and Noon–8 p.m. (‑2 days).

Responsibilities
  • Provide community outreach and oversee the welfare of a large caseload of clients.
  • Advocate for clients, ensuring quality of care and recommending appropriate resources.
  • Conduct assessments, address client needs, and make treatment recommendations.
  • Serve as primary liaison with families and guardians; collaborate closely with a multidisciplinary team of clinicians, nurses, peer specialists, and other providers.
  • Provide transportation, administer medication, and manage client finances as needed.
  • Document and update client health records at the Braintree office using an electronic health record system.
Education
  • High School diploma; BA in Psychology or a related field preferred.
  • One or more years of experience in the mental health field.
  • Working knowledge of community-based treatment, funding sources, and treatment regulations (strongly preferred).
Requirements
  • Current valid driver’s license in good standing with a good driving record; personal vehicle required for client transportation (fuel & mileage reimbursed).
  • CPR/First Aid and MAP certifications required.
Compensation

Salary Range: $22.40 – $25.25 per hour.
