The Community-Based Nurse Practitioner (NP) provides comprehensive, patient-centered supportive clinical care and wellness services to individuals and families in community & home settings. The NP role emphasizes outreach, prevention, health promotion, and support for Vitra clients with a focus on improving access to care for underserved populations in Adult Foster Care (AFC) programs. The NP collaborates with interdisciplinary teams, community organizations, and healthcare providers to ensure holistic, culturally sensitive, and evidence-based care.

Essential

Functions
  • Collaborates with community primary care providers to ensure clients optimal continuity of care.
  • Coordinates Care with providers, specialists, testing facilities and agencies.
  • Conducts health assessments, physical exams, and screenings in-home & in community-based settings (e.g., homes, community centers).
  • Develops, implements, and monitors individualized care plans to address clients’ AFC requirements and needs.
  • Monitors medications, therapies, and treatments in accordance with state regulations and scope of practice.
  • Provides clients and family education on disease prevention, lifestyle modification, medication adherence, and health promotion.
  • Coordinates referrals to specialists, social services, and community resources as needed.
  • Participates in outreach initiatives to promote community health & to engage hard-to-reach or vulnerable populations.
  • Provides clinical services in-home and communities, as required.
  • Document all care accurately and timely in the electronic health record (EHR).
  • Maintains current knowledge of evidence-based practices, regulatory standards, and community health trends.
  • Fosters culture of customer service and commitment to quality care.
  • Serves as a brand ambassador for Vitra reflecting our vision, mission, and values.
  • Shows a genuine interest and compassion for the communities we serve.
  • Provides telehealth support as needed.
  • Other duties as assigned.
Education, Experience, and Licensure Requirements
  • Masters or Doctorate degree in Nursing from an accredited program.
  • Current Nurse Practitioner license and board certification (FNP, AGNP, or other applicable specialties).
  • CPR certification.
  • Active state licensure and prescriptive authority (DEA license as required).
  • Minimum of 2–3 years of experience as a Nurse Practitioner required.
  • Strong clinical assessment, diagnostic, and decision-making skills.
  • Excellent communication and interpersonal skills with diverse populations.
  • Commitment to equity, inclusion, and culturally competent care.
  • Ability to work independently and collaboratively in a variety of community settings.
  • Bilingual is strongly preferred.
Physical and Environmental Demands
  • Works in a clean, well-lit environment with fluctuating temperatures in close proximity to others.
  • Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.
  • Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to navigate stairs; requires visual acuity and manual dexterity to operate equipment.
