Epic Nursing Services is seeking a motivated, qualified and experienced full-time nurse who is flexible, adaptable and, above all, patient in all types of situations. In this position, the selected candidate will do a combination of field and administrative work. A portion of the time will be spent in member homes, completing wellness and supervisory visits, MDS-HC assessments and re‑assessments. The other part of the time will be spent completing documentation, making follow‑up calls, and abiding by processes.
Nurses must be located within 30 miles of the designated coverage territory with transportation to members home been catered for. We are currently hiring a community nurse to case manage about 80‑150 members in the following areas:
Fall River, New Bedford, Brockton, Dorchester, Roxbury Quincy, and Malden. This position is mainly no hands‑on clinical services, with no nights, no weekends, or holiday shifts. This position is ideal for a nurse with home and community‑based experience in the rehabilitation field of the elderly and people with disabilities desiring to live independently in the community.
- Complete and coordinate all applicable clinical assessments and re‑assessments and clinical evaluations.
- Complete all required prior authorization and track expiring authorizations.
- Develop and review on an ongoing basis each member’s plan of care.
- Review the physician orders, medications and diagnosis.
- Monitor the health status of all members to ensure that all ordered services are properly delivered;
- Report changes in the health status of any member to the member’s primary physician.
- Review direct care aide and caregivers' documentation as directed.
- Conduct on‑site home visits with each member enrolled in accordance with the visit cycle policy.
- Complete a progress note corresponding with each on‑site visit or encounter, or more often as the member’s condition warrants.
- Submit semi‑annual health‑status reports to the member’s PCP.
- Plan for and implement discharges and transition from the agency programs.
- Conduct an orientation for each Direct Care Aide or caregiver before they can begin care provision.
- Provide ongoing training to Direct Care Aides and caregivers on health and aging
- Contact with members primary care physician to obtain medical information and physician clearances
- Other responsibilities as assigned by the Agency.
- Must be a graduate from a School of Nursing.
- Must be licensed in the State as a Registered Nurse or a Practical Nurse.
- Ability to travel throughout Massachusetts, mode of transportation provided.
- Minimum two years of experience working with elderly or disabled individuals.
- Excellent organizational, problem solving/planning and communication skills
- Demonstrated skills in Windows, Microsoft Word, Excel, Access and Outlook.
- Must be able to contribute to the quality of care being rendered through constructive communication with administrator and staff.
- Must have a criminal background check.
- Must have a current CPR certification.
