Preventative Care Community Health Worker

LAMP Community Health Centre seeks a Community Health Worker to enhance health prevention initiatives for Black, African and Caribbean communities in Peel Region. This position involves proactive outreach and culturally affirming care.

As a Community Health Worker within the BHSS Hub, you will focus on developing, implementing, and evaluating health programs tailored to BAC communities. Collaborate with team members and local organizations to enhance service accessibility while maintaining accurate records and tracking program outcomes.

Key Responsibilities:

• Plan and implement health prevention initiatives

• Conduct outreach and relationship building

• Support clients with health-related needs

• Contribute to interdisciplinary teamwork efforts

• Ensure client records are maintained in the EMR

Requirements:

• Minimum of 2 years relevant community healthcare experience

• Relevant diploma or degree in health or social services

• Proven experience in community outreach

• Strong organizational and prioritization skills

• Familiarity with the Peel region is preferred
