The Patient Care Coordinator serves as the first point of contact for patients, addressing inquiries while facilitating the opioid agonist therapy treatment program. Essential skills include working with individuals facing addiction, coordinating treatment plans, and utilizing Electronic Medical Records effectively. Make a difference in patients' recovery journeys through collaborative efforts with the healthcare team.
Key Responsibilities:
• Collaborate with a multi-disciplinary team for treatment plans
• Supervise urine drug screenings and testing
• Perform venipuncture and ECGs if trained
• Manage patient scripts and cancellations per protocols
• Maintain competence in Electronic Medical Records use
Requirements:
• Commitment to addiction and mental health support
• High School Diploma or GED required
• Experience in customer service or with vulnerable populations
• First Aid and CPR Certification essential
• Proficient in Microsoft Office applications
