As an entry-level Nurse Case Manager, you'll collaborate closely with physicians, assisting them in managing patient care and securing timely access to treatments. Your presence will significantly improve health outcomes for patients enrolled in the patient support program (PSP).
Key Responsibilities:
• Facilitate enrolment and reimbursement processes for patients
• Educate healthcare providers on various support services
• Handle documentation and authorization forms for medications
• Liaise among stakeholders to ensure service continuity
• Regularly track and report on patient support metrics
Requirements:
• Graduate of a Registered Nursing Program with licensure in Ontario
• Minimum of 2 years in Patient Support Programs
• Strong organizational skills and attention to detail
• Familiarity with PIPEDA in a patient care environment
• High proficiency with relevant digital tools and applications
