Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) partners with agencies in Peel and Dufferin to offer services at the Homeless and Addictions Recovery Treatment (HART) Hub. HART Hubs are community-based care hubs addressing mental health, addiction and housing needs for people experiencing homelessness. They provide integrated care, including primary care, mental health services, addiction care and employment supports.

Role:
Nurse Practitioner – HART Hub Brampton
The Nurse Practitioner will work with the Rapid Access Addiction Medicine (RAAM) team as an integrated member of the HART Hub. They will assess and treat symptoms of substance use and facilitate connections to continuum of care to increase access to community-based services addressing social determinants of health.
Status: Permanent part-time, 18.75 hours per week, daytime shifts with flexibility for evenings/weekends.

this posting is for an active vacancy. There is also potential for full-time where the position would be split between Brampton and Dufferin.
Salary: $55,276.59 – $65,031.29 annually, based on part-time hours. Salary may be adjusted based on qualifications.
Location: Based at 10 Peel Centre Drive, Brampton. The role is on-site and/or community-based due to program needs.
What You Will Do
- Attend to clients with addictions and concurrent disorders at the clinic, with or without scheduled appointments, and in the community via home visits during regular and extended hours.
- Perform comprehensive health or symptom-directed assessments within the RN (EC) scope and synthesize data to inform diagnoses.
- Spend time with clients to identify existing and potential health problems, focusing on substances used, related lifestyle, psychological, sociocultural and environmental risk factors, and coordinate referrals as needed.
- Obtain physician consultation as appropriate, per professional guidelines.
- Collaborate with physicians, specialists and HART Hub partners, and RAAM stakeholders.
- Determine the need for and order appropriate diagnostic tests, interpret results within scope of practice.
- Prescribe drugs within the RN (EC) scope where permitted.
- Monitor ongoing therapy for clients with addictions or chronic conditions through pharmacological, counseling or supplementary interventions.

What You Need to Join Our Team
- Master of Science in Nursing and Registered Nurse Practitioner (RN-EC) in good standing with the College of Nurses of Ontario.
- Minimum 2 years of experience in mental health and/or addictions in a community setting.
- Completed College Approved Controlled Substances Education Program.
- Psychiatric Nursing Certification is an asset.
- Knowledge and experience with people experiencing problematic substance misuse and related health issues.
- Experience with dual diagnosis or concurrent disorders is an asset.
