Well Fort is seeking a full-time Registered Nurse to coordinate intake and navigation for clients across various services. The RN will apply trauma-informed care and work collaboratively with interprofessional teams to ensure seamless access to health, social, and community services. This position focuses on holistic assessments and developing care plans for clientele, particularly those facing marginalization or barriers to access.
Key Responsibilities:
• Perform central intake and holistic assessments of clients
• Develop individualized prevention and care management plans
• Coordinate and monitor delivery of care across programs
• Provide health education and ongoing support to clients
• Maintain accurate EMR documentation for all client interactions
Requirements:
• Bachelor’s degree in nursing and in good standing
• Experience in care coordination with marginalized populations
• Strong understanding of trauma-informed and harm reduction practices
• Excellent communication skills and ability to multitask
• Valid driver's license and current First Aid/CPR certification
