Social Services Worker - Resident & Family Coordinator

As the Resident & Family Experience Coordinator II, you will lead important transitions for residents, provide essential counseling services, and develop initiatives that foster support for families. Your engagement with residents will have a genuine impact on their quality of life, promoting connectivity and well-being.
Key Responsibilities:
• Ensure smooth transitions for residents in various stages
• Offer counseling services as per qualifications
• Create and evaluate individualized social work services
• Facilitate family involvement in advanced care planning
• Manage feedback to improve resident satisfaction
Requirements:
• Must be a Registered Social Services Worker
• At least three years in a related field
• Familiarity with the Long Term Care Act is a plus
• Excellent communication and planning skills
• Experience in customer service is advantageous
